• Please bring your insurance card and the script written by your doctor (if you have one).
  
• Please download the attached four forms and have them filled out when you arrive for your first appointment OR arrive 10-15 minutes early to fill out this paperwork in our office.
  
• Please dress appropriately (if you are being seen for a knee injury please wear or bring shorts)
